Eswatini’s commitment to malaria elimination by 2028 offers critical insights for public health practitioners and policymakers across sub-Saharan Africa. The country has successfully transitioned from endemic transmission to a low-burden setting where most remaining cases are imported—demonstrating that elimination is achievable when domestic transmission control is prioritized through sustained surveillance, vector management, and treatment access.

Three practical lessons emerge from Eswatini’s trajectory: first, elimination requires comprehensive long-term commitment beyond emergency response; second, imported cases become the primary elimination barrier once domestic control is established, necessitating border health strategies; and third, 47 nations already certified malaria-free provide proven blueprints for implementation. For health systems and governments in endemic regions, Eswatini’s progress signals that ambitious elimination targets are evidence-based and feasible when combined with adequate funding, skilled workforce development, and robust monitoring systems. This pathway offers a replicable model for regional malaria control acceleration.
