A major systematic review has delivered important clinical implications for bone health management in older adults. The evidence now suggests that calcium and vitamin D supplements alone provide limited fracture prevention benefits in healthy older populations, challenging the assumption that universal supplementation is cost-effective for all patients. This finding does not eliminate supplementation entirely but rather emphasizes the need for targeted, individualized approaches. Clinicians and patients should consider that exercise programs, adequate protein intake, and structured fall prevention strategies may offer superior outcomes for maintaining bone health and preventing fractures. Rather than relying solely on supplementation, a comprehensive approach incorporating multiple modalities—physical activity, nutritional optimization, and environmental safety measures—appears better aligned with current evidence. Older adults should discuss personalized bone health strategies with their healthcare providers, particularly those with specific fracture risk factors who may still benefit from targeted supplementation.
