A new study in the Journal of Cachexia, Sarcopenia and Muscle provides critical guidance for clinicians and aging individuals seeking to preserve strength and independence. First, strength should be understood as a neural trait that precedes muscular capacity—motoneuron excitability sets the physiological ceiling for force production, making neural health paramount. Second, master athletes demonstrate conclusively that aging-related decline is not inevitable; sustained physical engagement can preserve neural function across decades. Third, resistance and high-intensity training work through a dual mechanism, protecting both nerve function and muscle tissue simultaneously, making them essential components of aging well. These findings shift the clinical paradigm from viewing sarcopenia as purely muscular to recognizing it as fundamentally a neuromuscular disorder. Healthcare providers should emphasize exercise interventions targeting both neural and muscular systems in prevention and treatment strategies.
