New data clarifies a common point of confusion in vitamin D therapeutics: the timeframe required to achieve steady-state blood levels. Regardless of whether patients receive vitamin D as a daily 1,500 IU dose, weekly 10,500 IU bolus, or monthly 45,000 IU administration, circulating 25-hydroxyvitamin D stabilises within 8–12 weeks. This uniform timeline reflects vitamin D’s extended half-life of 15–25 days in circulation. A landmark randomised trial of 48 elderly patients with hip fracture confirmed that total weekly dose—not dosing frequency—determines final serum concentrations. Clinicians should counsel patients that apparent dosing differences mask equivalent pharmacological outcomes. The practical implication: weekly regimens deserve preference solely because they enhance adherence and reduce pill burden, streamlining treatment success without sacrificing efficacy.
