First, gepants function as calcitonin gene-related peptide (CGRP) antagonists, blocking a key neurochemical implicated in migraine initiation and the associated neuroinflammatory cascade. This mechanism of action represents a departure from conventional migraine therapies and may benefit patients who have not responded adequately to traditional treatments.
Second, the availability of multiple formulation routes—both oral and intranasal—allows tailored treatment approaches suited to individual patient needs and preferences, supporting both acute symptom management and preventive therapy. Third, and perhaps most importantly, the demonstrated efficacy and tolerability of gepants expand the therapeutic armamentarium for vestibular migraine, offering hope to patients whose quality of life has been significantly compromised by the combined burden of headache, dizziness, and balance dysfunction.
