The Framingham Heart Study’s latest analysis offers three critical insights for understanding vitamin D’s potential role in Alzheimer’s disease prevention. First, higher vitamin D levels in midlife appear protective specifically against tau accumulation in the entorhinal cortex and parahippocampal gyrus—the anatomical sites where Alzheimer’s pathology begins—suggesting a targeted neuroprotective mechanism. Second, the protective effect is exclusive to tau pathology and does not extend to amyloid-beta plaques, indicating that vitamin D may influence distinct pathological pathways rather than providing broad neuroprotection. Third, and importantly, this remains a correlational finding from an observational study; causation cannot yet be established. While these findings are encouraging for future preventive strategies, patients and clinicians should recognize that vitamin D supplementation for Alzheimer’s prevention is not yet evidence-based practice. Randomized controlled trials specifically testing supplementation efficacy are essential before clinical recommendations can be implemented.
