What is Azathioprine?
Azathioprine is an immunosuppressive medication that belongs to a class of drugs called antimetabolites. It works by suppressing the immune system and is primarily used to prevent organ transplant rejection and treat certain autoimmune diseases. This medication has been a cornerstone of immunosuppressive therapy for decades and requires careful monitoring due to its potent effects on the immune system.
Uses
Azathioprine is prescribed for several medical conditions:
- Organ transplant rejection prevention: Used in combination with other immunosuppressants to prevent the body from rejecting transplanted kidneys, liver, or heart
- Rheumatoid arthritis: Prescribed when other treatments have failed to control joint inflammation and damage
- Inflammatory bowel disease: Helps maintain remission in Crohn’s disease and ulcerative colitis
- Autoimmune hepatitis: Used to reduce liver inflammation and prevent progression of the disease

How it works
Azathioprine works by interfering with DNA and RNA synthesis in rapidly dividing cells, particularly immune cells called lymphocytes. The drug is converted in the body to compounds that block the production of purines, which are essential building blocks for genetic material. By disrupting this process, azathioprine reduces the activity of T and B cells, effectively dampening the immune response that can cause transplant rejection or autoimmune disease symptoms.
Side effects
Common side effects include:
- Nausea and vomiting
- Loss of appetite
- Fatigue and weakness
- Hair loss
- Increased susceptibility to infections
- Muscle aches
- Diarrhea
- Skin rash
Serious side effects to watch for:
- Signs of bone marrow suppression (unusual bruising, bleeding, or persistent infections)
- Liver toxicity (yellowing of skin or eyes, dark urine, severe abdominal pain)
- Increased risk of certain cancers, particularly skin cancer and lymphoma
- Severe allergic reactions
- Pancreatitis (severe abdominal pain, nausea, vomiting)

- Regular monitoring required: Blood tests must be performed regularly to check for bone marrow suppression and liver function
- Infection risk: Patients should avoid live vaccines and contact with people who have infections, as the medication suppresses immune function
- Cancer screening: Regular skin examinations are recommended due to increased skin cancer risk, especially with sun exposure
- Genetic testing consideration: Some patients with certain genetic variations may require dosage adjustments or alternative treatments
Interactions
- Allopurinol: Significantly increases azathioprine levels, requiring dose reduction to prevent toxicity
- ACE inhibitors: May increase risk of severe blood disorders when combined with azathioprine
- Warfarin: Azathioprine may reduce the effectiveness of warfarin, requiring closer monitoring of blood clotting
- Live vaccines: Should be avoided as azathioprine may prevent proper immune response and increase infection risk
- Trimethoprim-sulfamethoxazole: May increase risk of blood disorders
- Ribavirin: Combination may increase risk of severe anemia
- Methotrexate: Combined use may increase risk of liver toxicity

Dosage
Azathioprine dosing varies significantly based on the condition being treated and individual patient factors. For transplant recipients, initial doses typically range from 3-5 mg per kilogram of body weight daily, often reduced to 1-3 mg/kg daily for maintenance. For autoimmune conditions like rheumatoid arthritis, doses usually start at 1 mg/kg daily and may be increased gradually to 2.5 mg/kg daily based on response and tolerance. The medication is usually taken once or twice daily with food to reduce stomach upset. Dosage adjustments are frequently necessary based on blood test results and response to treatment.
