What is Temazepam?

Temazepam has several documented medical uses:
- Insomnia treatment: The primary indication for temazepam is short-term management of sleep disorders, helping patients fall asleep faster and maintain sleep throughout the night.
- Pre-operative sedation: Sometimes used as a sedative before surgical procedures to reduce anxiety and promote relaxation.
- Anxiety-related sleep disturbances: May be prescribed for sleep problems specifically caused by anxiety disorders when other treatments are insufficient.
- Hospital-based sleep management: Used in clinical settings to help patients sleep in unfamiliar or stressful environments.

How it works
Temazepam works by enhancing the activity of gamma-aminobutyric acid (GABA), the brain’s primary inhibitory neurotransmitter. It binds to specific GABA receptors in the brain, increasing the frequency of chloride channel opening, which results in enhanced neuronal inhibition. This action produces sedative, hypnotic, and muscle relaxant effects that promote sleep onset and maintenance.

Warnings and precautions
- Pregnancy and breastfeeding: Temazepam is classified as pregnancy category X, meaning it should not be used during pregnancy due to risk of birth defects and withdrawal symptoms in newborns. It is also not recommended during breastfeeding.
- Elderly patients: Older adults are more sensitive to benzodiazepines and face increased risk of falls, cognitive impairment, and prolonged sedation, requiring lower doses and careful monitoring.
- Respiratory conditions: Patients with sleep apnea, chronic obstructive pulmonary disease, or other breathing disorders should use temazepam with extreme caution due to risk of respiratory depression.
- Substance abuse history: Individuals with a history of alcohol or drug abuse face higher risk of developing dependence and should be closely monitored.
- Liver and kidney impairment: Patients with hepatic or renal dysfunction may require dose adjustments and more frequent monitoring due to altered drug metabolism and elimination.
Interactions
- Alcohol: Significantly increases sedation and respiratory depression risk; concurrent use should be avoided completely.
- Opioid medications: Combination greatly increases risk of severe sedation, respiratory depression, and potentially fatal overdose.
- Other benzodiazepines: Concurrent use amplifies sedative effects and increases risk of excessive central nervous system depression.
- Antihistamines: Medications like diphenhydramine can enhance drowsiness and cognitive impairment when combined with temazepam.
- Barbiturates: Increase sedative effects and may lead to dangerous levels of central nervous system depression.
- Antidepressants: Some antidepressants, particularly tricyclics and MAO inhibitors, can enhance temazepam’s sedative effects.
- Muscle relaxants: Combination may result in excessive muscle weakness and increased fall risk.
- St. John’s Wort: This herbal supplement may reduce temazepam effectiveness by increasing its metabolism.

Dosage
For adults, the typical starting dose of temazepam is 15 mg taken orally 30 minutes before bedtime. The dose may be adjusted based on individual response, with a usual range of 7.5 mg to 30 mg per night. Elderly patients typically start with 7.5 mg due to increased sensitivity. Treatment should generally be limited to 7-10 days, and the medication should be taken only when a full night’s sleep (7-8 hours) is possible.
