99.5% Donor Incompatibility Reversed: The Power of CAR T in Transplantation

A groundbreaking clinical trial reveals the remarkable impact of CAR T-cell therapy on donor compatibility rates. One patient had developed antibodies against 99.5% of potential kidney donors, while another had antibodies against 99.9%—rates far exceeding the 85% rejection typical among highly sensitized patients. These extreme cases represented some of the most challenging transplant matches in the nation.

Following CAR T-cell treatment, both patients successfully received kidney transplants, demonstrating the therapy’s ability to effectively neutralize antibody-producing B cells. This dramatic shift in compatibility—from nearly universal rejection to successful transplantation—highlights the transformative potential of applying cancer immunotherapy techniques to solid organ transplantation.

The data underscores how CAR T technology could reshape outcomes for the approximately 20% of transplant candidates who are highly sensitized and face extended waiting periods.
