Vitamin B12 deficiency produces fatigue and cognitive decline that mimic normal ageing, yet often goes undiagnosed until neurological damage occurs. A striking finding from clinical B12 deficiency literature reveals that unexplained fatigue affects 92 percent of undiagnosed B12-deficient patients, yet many receive reassurance that their symptoms represent inevitable age-related decline. This normalization of symptoms delays diagnosis and treatment, allowing progressive mitochondrial dysfunction to advance unchecked.

Functional metabolic screening using markers such as methylmalonic acid and homocysteine can identify cellular B12 insufficiency before serum levels fall, enabling early intervention when symptoms remain reversible. Beyond fatigue, patients frequently present with cognitive slowing (78%), mood disturbance (64%), and peripheral neuropathy (58%)—all attributable to B12’s essential role in mitochondrial energy production and neurological integrity. The clinical message is clear: fatigue should not be dismissed as normal ageing without appropriate biochemical investigation.
