A major 15-year study of 12,047 paracetamol poisoning cases in New South Wales has exposed a critical gap in post-overdose care: while severe liver injury remains uncommon at 4.7%, nearly one in four patients return to hospital for repeat overdose attempts within five years.

The PAVLOVA-3 study, published in The Lancet Regional Health – Western Pacific, found that 85.2% of poisonings were intentional, with women aged 15-24 years representing the highest-risk demographic. Most concerning is the elevated mortality risk that persists for 15 years following initial admission, suggesting that current hospital protocols addressing acute toxicity fail to address underlying mental health needs.

These findings underscore an urgent clinical imperative: paracetamol poisoning admissions should trigger comprehensive psychiatric evaluation and long-term mental health follow-up, not merely toxicological management.
