Vitamin B12, folate, and B6 do not function in isolation. Rather, they operate as an interdependent metabolic system regulating one-carbon metabolism—a biochemical pathway essential for DNA synthesis, red blood cell formation, neurotransmitter balance, and cardiovascular health. Each vitamin plays a distinct role: B12 facilitates the conversion of homocysteine to methionine, folate provides the methyl groups for this reaction, and B6 supports transsulfuration pathways that manage homocysteine levels. When clinicians or patients supplement based on a single deficiency without assessing the complete triad, metabolic imbalances can emerge, potentially affecting multiple organ systems. This interconnected system underscores why comprehensive laboratory assessment precedes evidence-based supplementation. Targeted dosing based on individual biochemical status—not marketing claims—should guide therapeutic decisions.
