A landmark study published in The New England Journal of Medicine has exposed a critical gap in clinical practice: nearly one-third of patients suffering from B12-related neurological damage show completely normal results on routine blood tests. This finding challenges decades of conventional screening protocols and suggests that millions of patients may be developing preventable nerve damage while their standard laboratory markers appear reassuring.

The research reveals that B12 operates through two separate cellular pathways—one affecting red blood cell formation, another protecting nerve myelin. Standard complete blood count tests only detect dysfunction in the first pathway. Clinicians relying solely on conventional serum B12 and CBC screening are missing functional deficiency cases that progress silently. The study demonstrates that methylmalonic acid testing achieves 98.4% specificity in confirmed B12 deficiency, making it a far superior diagnostic tool.
