Contemporary evidence-based practice in chronic GVHD management rests on three fundamental principles that guide clinical decision-making. First, accurate diagnosis demands integration of multiple diagnostic modalities—clinical assessment, histopathological examination, and organ-specific testing—rather than reliance on any single diagnostic test. This comprehensive approach ensures earlier identification and appropriate disease characterization across diverse presentations.

Second, treatment strategies have evolved substantially, spanning traditional immunosuppressive agents, emerging JAK inhibitors, and organ-specific supportive interventions tailored to individual patient phenotypes. Third, and critically, quality-of-life outcomes and long-term survival data remain inadequately studied in cGVHD populations. Healthcare systems must prioritize development of robust long-term follow-up infrastructure and standardized outcome assessments to better understand treatment efficacy and optimize patient-centered care strategies in this vulnerable transplant survivor population.
