The UK National Screening Committee has published two detailed position statements to explain the rigorous evidence framework underlying its screening recommendations. These documents address a critical gap between test availability and justified screening programmes. The committee’s evaluation framework examines test accuracy, treatment effectiveness, population benefit, and harm-benefit ratios as interconnected components rather than isolated metrics. By publishing these position statements, the committee provides stakeholders with transparent criteria for assessing both established and emerging screening proposals, particularly multicancer detection tests. The statements clarify that surrogate outcomes require careful interpretation and that population-wide benefits must be demonstrably achievable before implementing screening programmes. This evidence-driven approach aims to ground screening policy in clinical reality rather than technological possibility.
