New evidence demonstrates that genetic testing can meaningfully improve heart failure prevention strategies. A major study identifies three key clinical implications: first, dapagliflozin provides superior cardiovascular protection in patients carrying pathogenic variants linked to cardiomyopathy, reducing heart failure risk by 32%. Second, conventional medications including ACE inhibitors and beta blockers show notably reduced effectiveness in this genetically defined subgroup, suggesting these patients require alternative approaches. Third, the cost of genetic testing appears justified when considering hospitalization prevention and improved outcomes. The research, conducted by Mass General Brigham, enrolled over 12,800 patients and focused particularly on those with sarcomere gene variants. For clinicians managing high-risk populations and patients with family histories of sudden cardiac death or cardiomyopathy, genetic profiling followed by targeted dapagliflozin therapy may represent a meaningful advancement in preventive cardiology.
