Dulaglutide Drug Profile

What is Dulaglutide?

Dulaglutide is a prescription medication that belongs to a class of drugs called GLP-1 receptor agonists. It is primarily used to treat type 2 diabetes mellitus by helping to control blood sugar levels. The medication is administered as a once-weekly injection under the skin and works by mimicking the action of a natural hormone that regulates blood glucose.

Uses

Dulaglutide has several approved medical uses:

  • Type 2 diabetes management: The primary use is to improve blood sugar control in adults with type 2 diabetes, typically used alongside diet and exercise modifications.
  • Cardiovascular risk reduction: Approved to reduce the risk of major cardiovascular events such as heart attack, stroke, and cardiovascular death in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
  • Combination therapy: Often used in combination with other diabetes medications like metformin, sulfonylureas, or insulin when single-drug therapy is insufficient.
  • Weight management support: While not primarily a weight loss medication, dulaglutide often leads to modest weight reduction in patients with type 2 diabetes.
  • Beta-cell preservation: May help preserve pancreatic beta-cell function, which is responsible for insulin production.

How it works

Dulaglutide works by activating GLP-1 receptors in the body, mimicking the action of glucagon-like peptide-1, a hormone naturally produced in the intestines. This activation stimulates insulin release when blood sugar levels are elevated, suppresses glucagon secretion from the liver, and slows gastric emptying, which helps prevent post-meal blood sugar spikes. The medication also acts on the brain to promote feelings of satiety, which can contribute to weight loss.

  • Injection site reactions
  • Constipation

Serious side effects to watch for:

  • Severe allergic reactions including difficulty breathing or swallowing
  • Signs of pancreatitis such as severe abdominal pain that may radiate to the back
  • Symptoms of thyroid tumors including persistent hoarseness, difficulty swallowing, or neck lumps
  • Severe hypoglycemia, especially when used with insulin or sulfonylureas
  • Kidney problems or worsening of existing kidney disease

Warnings and precautions

  • Thyroid cancer risk: Dulaglutide carries a black box warning regarding potential thyroid C-cell tumors. Patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 should not use this medication.
  • Pancreatitis history: Individuals with a history of pancreatitis should avoid dulaglutide, as GLP-1 receptor agonists may increase the risk of acute pancreatitis.
  • Pregnancy and breastfeeding: The safety of dulaglutide during pregnancy has not been established. Women should discuss family planning with their healthcare provider, as the medication should be discontinued at least 2 months before a planned pregnancy.
  • Kidney function: Patients with severe kidney impairment should use dulaglutide with caution, as gastrointestinal side effects may worsen kidney function through dehydration.
  • Diabetic retinopathy: Rapid improvement in blood sugar control may temporarily worsen diabetic eye disease in some patients.

Interactions

  • Insulin and sulfonylureas: Increased risk of hypoglycemia; dosage adjustments may be necessary to prevent dangerously low blood sugar.
  • Oral medications: Dulaglutide slows gastric emptying, which may delay the absorption of oral drugs taken around the same time.
  • Warfarin: May affect the absorption and effectiveness of warfarin; INR monitoring may need to be increased.
  • Birth control pills: The delayed gastric emptying may affect the absorption of oral contraceptives.
  • Digoxin: Potential for altered absorption due to slowed gastric emptying.
  • Acetaminophen: May delay absorption, though total absorption typically remains unchanged.
  • Antibiotics: Timing of antibiotic administration may need adjustment to ensure proper absorption.
  • Thyroid medications: Absorption may be affected; monitor thyroid function tests more closely.

Dosage

The typical starting dose for dulaglutide is 0.75 mg administered once weekly as a subcutaneous injection. The dose may be increased to 1.5 mg once weekly if additional blood sugar control is needed. For patients requiring further glycemic control, the dose can be increased to 3 mg once weekly, and in some cases up to 4.5 mg once weekly. The injection can be given at any time of day, with or without meals, but should be administered on the same day each week. Dosage adjustments should always be made under medical supervision based on individual blood sugar control and tolerance.
