Liraglutide Drug Profile

What is Liraglutide?

Liraglutide is a synthetic hormone medication that belongs to a class of drugs called GLP-1 receptor agonists. It is primarily prescribed to help manage type 2 diabetes by improving blood sugar control and is also approved for weight management in certain patients. The medication mimics the action of a natural hormone called GLP-1 (glucagon-like peptide-1) that helps regulate blood glucose levels and appetite.

Uses

Liraglutide has several FDA-approved medical uses:

  • Type 2 diabetes management: Used alongside diet and exercise to improve glycemic control in adults with type 2 diabetes
  • Cardiovascular risk reduction: Helps reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease
  • Chronic weight management: Approved for weight loss in adults with obesity or overweight individuals with at least one weight-related health condition
  • Pediatric type 2 diabetes: Treatment option for children and adolescents aged 10 years and older with type 2 diabetes

How it works

Liraglutide works by activating GLP-1 receptors in the body, which triggers several beneficial effects for blood sugar control. It stimulates insulin release from the pancreas when blood glucose levels are elevated, while simultaneously reducing glucagon secretion that would otherwise raise blood sugar. Additionally, it slows gastric emptying and promotes feelings of fullness, which can lead to reduced food intake and weight loss.

  • Nausea and vomiting
  • Diarrhea
  • Decreased appetite
  • Constipation
  • Abdominal pain
  • Headache
  • Fatigue
  • Injection site reactions

Serious side effects to watch for:

  • Pancreatitis (severe abdominal pain that may radiate to the back)
  • Gallbladder problems including gallstones
  • Kidney problems or worsening of existing kidney disease
  • Severe allergic reactions
  • Thyroid tumors (rare but potentially serious)

Warnings and precautions

  • Thyroid cancer risk: Liraglutide carries a boxed warning for potential thyroid C-cell tumors and should not be used in patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2
  • Pancreatitis history: Patients with a history of pancreatitis should avoid this medication due to increased risk of recurrence
  • Pregnancy considerations: Limited data exists on use during pregnancy; potential benefits must be weighed against potential risks to the fetus
  • Kidney function: Use with caution in patients with kidney impairment, as dehydration from gastrointestinal side effects may worsen kidney function
  • Diabetic retinopathy: Rapid improvement in glucose control may temporarily worsen diabetic eye disease in some patients

Interactions

  • Insulin and sulfonylureas: Increased risk of hypoglycemia when combined; dosage adjustments may be necessary
  • Oral medications: May delay absorption of oral drugs due to slowed gastric emptying; timing of administration may need adjustment
  • Warfarin: Potential for altered INR levels requiring more frequent monitoring
  • Digoxin: May affect digoxin absorption and require monitoring of levels
  • Acetaminophen: Absorption may be delayed but overall effectiveness typically unchanged
  • Oral contraceptives: Timing of administration may need to be adjusted to ensure effectiveness
  • Antibiotics: Absorption of oral antibiotics may be delayed
  • Levothyroxine: May require monitoring of thyroid hormone levels and potential dose adjustments

Dosage

For type 2 diabetes, liraglutide is typically started at 0.6 mg once daily by subcutaneous injection, with increases to 1.2 mg daily after one week if tolerated. The dose may be further increased to 1.8 mg daily if additional glycemic control is needed. For weight management, the dosing schedule involves gradual escalation over several weeks, starting at 0.6 mg daily and potentially increasing up to 3.0 mg daily based on tolerability and response. The medication is administered as a once-daily injection under the skin of the thigh, abdomen, or upper arm. Dosage should always be determined and monitored by a qualified healthcare provider who can assess individual patient factors and treatment response.
