Levofloxacin: A Comprehensive Drug Profile
What is Levofloxacin?
Levofloxacin is a broad-spectrum antibiotic belonging to the fluoroquinolone class of medications. It is used to treat a wide range of bacterial infections by inhibiting bacterial DNA replication and repair. This prescription medication is available in oral tablet, oral solution, and intravenous formulations for various types of infections.
Uses
Levofloxacin is prescribed for several types of bacterial infections:
- Respiratory tract infections: Including community-acquired pneumonia, acute bacterial sinusitis, and acute bacterial exacerbations of chronic bronchitis
- Urinary tract infections: Both complicated and uncomplicated infections of the bladder and kidneys, including pyelonephritis
- Skin and soft tissue infections: Treatment of complicated infections involving skin, subcutaneous tissue, and muscles
- Prostatitis: Chronic bacterial prostatitis in men when caused by susceptible organisms
- Inhalational anthrax: Post-exposure prophylaxis and treatment following suspected or confirmed exposure to Bacillus anthracis
How it works
Levofloxacin works by targeting essential bacterial enzymes called DNA gyrase and topoisomerase IV, which are necessary for bacterial DNA replication, transcription, and repair. By inhibiting these enzymes, the medication prevents bacteria from reproducing and ultimately leads to bacterial cell death. This mechanism is specific to bacterial cells and does not significantly affect human DNA processes.
Side effects
Common side effects include:
- Nausea and vomiting
- Diarrhea
- Headache
- Dizziness
- Trouble sleeping (insomnia)
- Stomach pain or discomfort
- Constipation
- Fatigue or weakness
Serious side effects requiring immediate medical attention:
- Tendon pain, swelling, or rupture (especially Achilles tendon)
- Severe diarrhea that may indicate Clostridioides difficile infection
- Signs of peripheral neuropathy (tingling, numbness in extremities)
- Irregular heartbeat or QT prolongation
- Signs of liver damage (yellowing of skin or eyes, dark urine)
Warnings and precautions
- Tendon damage risk: Fluoroquinolones carry an increased risk of tendinitis and tendon rupture, particularly in patients over 60, those taking corticosteroids, or individuals with kidney, heart, or lung transplants
- Pregnancy and breastfeeding: Generally avoided during pregnancy and breastfeeding unless benefits clearly outweigh risks, as it may affect fetal cartilage development
- Kidney function: Dosage adjustment is necessary in patients with reduced kidney function, as the drug is primarily eliminated through the kidneys
- Mental health effects: May cause central nervous system effects including confusion, depression, hallucinations, and increased risk of suicidal thoughts
- Myasthenia gravis: Can worsen muscle weakness in patients with this condition and should be avoided
Interactions
- Antacids and supplements: Calcium, magnesium, aluminum, iron, and zinc can significantly reduce levofloxacin absorption when taken simultaneously
- Warfarin: May enhance anticoagulant effects, increasing bleeding risk and requiring more frequent INR monitoring
- NSAIDs: Combined use may increase risk of central nervous system stimulation and seizures
- Corticosteroids: Concurrent use increases the risk of tendon rupture, especially in elderly patients
- Antidiabetic medications: May cause unpredictable blood sugar changes, requiring closer glucose monitoring
- QT-prolonging drugs: Medications like certain antiarrhythmics, antipsychotics, and antidepressants may increase risk of dangerous heart rhythm changes
- Theophylline: May increase theophylline levels, potentially leading to toxicity
- Cyclosporine: May increase cyclosporine levels and associated kidney toxicity risk

Dosage
Typical adult dosages vary based on the type and severity of infection. Common regimens include 250-750 mg once daily by mouth or intravenously, with treatment duration ranging from 3-14 days depending on the condition being treated. For urinary tract infections, doses are often 250-500 mg daily, while respiratory infections may require 500-750 mg daily. Dosage adjustments are necessary for patients with kidney impairment. All dosing should be individualized and determined by a healthcare provider based on the specific infection, patient factors, and local resistance patterns.
