What is Trandolapril?
Trandolapril is a prescription medication belonging to a class of drugs called angiotensin-converting enzyme (ACE) inhibitors. It is primarily used to treat high blood pressure (hypertension) and heart failure by helping blood vessels relax and widen. This medication is taken orally and works by blocking certain enzymes that cause blood vessels to narrow.

Trandolapril is prescribed for several cardiovascular conditions:
- High blood pressure (hypertension): Helps lower blood pressure by relaxing blood vessels, reducing the risk of stroke, heart attack, and kidney problems
- Heart failure: Improves heart function and reduces symptoms in patients with reduced heart pumping ability

How it works
Trandolapril blocks the action of angiotensin-converting enzyme (ACE), which normally converts angiotensin I to angiotensin II, a powerful substance that causes blood vessels to narrow and triggers the release of hormones that increase blood pressure. By inhibiting this enzyme, trandolapril allows blood vessels to remain relaxed and dilated, which lowers blood pressure and reduces the workload on the heart. This mechanism also helps improve blood flow to the kidneys and other organs.
Side effects
Common side effects:
- Dry, persistent cough
- Dizziness or lightheadedness
- Fatigue or weakness
- Headache
- Nausea
- Low blood pressure (hypotension)
- Elevated potassium levels (hyperkalemia)
- Decreased kidney function
Serious side effects requiring immediate medical attention:
- Swelling of face, lips, tongue, or throat (angioedema)
- Severe kidney problems or kidney failure
- Dangerously high potassium levels causing heart rhythm problems
- Severe drop in blood pressure causing fainting
- Liver problems with yellowing of skin or eyes
Warnings and precautions
- Pregnancy: Trandolapril is contraindicated during pregnancy as it can cause serious harm to the developing fetus, including kidney problems, low blood pressure, and death. Women should use effective contraception while taking this medication.
- Kidney disease: Patients with existing kidney problems require careful monitoring as ACE inhibitors can worsen kidney function, especially in those with severe kidney disease or bilateral renal artery stenosis.
- High potassium levels: Regular blood tests are needed to monitor potassium levels, as trandolapril can cause dangerous elevations, particularly in patients with kidney disease or diabetes.
- Surgery and anesthesia: Patients should inform their healthcare providers about trandolapril use before any surgical procedures, as it may interact with anesthesia and cause severe low blood pressure.
- Dehydration: Patients should maintain adequate fluid intake and avoid situations that may cause dehydration, as this can lead to dangerous drops in blood pressure.
Interactions
- Potassium supplements and salt substitutes: Can cause dangerous elevations in blood potassium levels when combined with trandolapril
- Diuretics (water pills): May cause excessive blood pressure lowering, especially when starting treatment
- NSAIDs (ibuprofen, naproxen): Can reduce the effectiveness of trandolapril and increase the risk of kidney problems
- Lithium: Trandolapril can increase lithium levels in the blood, potentially leading to lithium toxicity
- Diabetes medications: ACE inhibitors may enhance the blood sugar-lowering effects of diabetes drugs, increasing the risk of hypoglycemia
- Other blood pressure medications: Combined use may cause excessive blood pressure reduction requiring dose adjustments
- Aliskiren: Should not be used together in patients with diabetes or kidney disease due to increased risk of kidney problems and high potassium
- Gold injections: May cause severe reactions including flushing, nausea, and dangerously low blood pressure

Dosage
Typical adult dosages for trandolapril vary based on the condition being treated. For hypertension, the usual starting dose is 1-2 mg once daily, which may be increased gradually to a maximum of 8 mg daily based on blood pressure response. For heart failure, treatment typically begins with 1 mg once daily, with gradual increases up to 4 mg daily as tolerated. Patients with kidney impairment may require lower starting doses.
