Haloperidol
What is Haloperidol?
Haloperidol is a typical antipsychotic medication belonging to the butyrophenone class of drugs. It is primarily prescribed to treat schizophrenia, acute psychosis, and severe behavioral problems, and is also used to control tics and vocal outbursts associated with Tourette syndrome. This medication has been in clinical use since the 1950s and remains an important treatment option for various psychiatric and neurological conditions.
Uses
Haloperidol is approved for several medical conditions:
- Schizophrenia: Helps manage symptoms such as hallucinations, delusions, and disorganized thinking in both acute episodes and long-term maintenance treatment.
- Acute psychosis: Provides rapid control of severe agitation, aggression, and psychotic symptoms in emergency situations.
- Tourette syndrome: Reduces motor and vocal tics when they significantly interfere with daily functioning.
- Severe behavioral disorders: Treats aggressive behavior and severe conduct problems in children and adults when other treatments have failed.
- Delirium: Sometimes used in hospital settings to manage severe agitation associated with delirium, though this use requires careful consideration of risks and benefits.
How it works
Haloperidol works by blocking dopamine receptors in the brain, particularly the D2 subtype of dopamine receptors. This action helps reduce the excessive dopamine activity that is thought to contribute to psychotic symptoms and abnormal movements. By normalizing dopamine transmission in specific brain regions, haloperidol can effectively control hallucinations, delusions, and other symptoms of psychosis.
Side effects
Common side effects include:
- Drowsiness and sedation
- Dry mouth
- Constipation
- Blurred vision
- Dizziness or lightheadedness
- Weight gain
- Muscle stiffness or tremor
- Restlessness or inability to sit still
Serious side effects that require immediate medical attention:
- Tardive dyskinesia (involuntary facial and body movements)
- Neuroleptic malignant syndrome (high fever, muscle rigidity, confusion)
- Severe allergic reactions
- Irregular heartbeat or QT prolongation
- Severe muscle spasms or dystonia
Warnings and precautions
- Elderly patients with dementia: Haloperidol carries an increased risk of death when used in elderly patients with dementia-related psychosis and is not approved for this indication.
- Pregnancy and breastfeeding: Should only be used during pregnancy when potential benefits justify potential risks to the fetus. The drug passes into breast milk and may affect nursing infants.
- Heart conditions: Patients with heart rhythm disorders, recent heart attack, or electrolyte imbalances require careful monitoring as haloperidol can affect heart rhythm.
- Liver and kidney function: Dose adjustments may be necessary in patients with liver impairment, as the drug is metabolized by the liver.
- Parkinson’s disease: Should be avoided in patients with Parkinson’s disease as it can worsen motor symptoms due to dopamine blockade.
Interactions
Haloperidol has several important drug interactions:
- Central nervous system depressants: Alcohol, benzodiazepines, and opioids can increase sedation and respiratory depression when combined with haloperidol.
- Anticholinergic medications: Drugs like atropine or tricyclic antidepressants can worsen side effects such as dry mouth, constipation, and confusion.
- Lithium: Combination may increase risk of neurological side effects and should be monitored closely.
- Medications affecting heart rhythm: Drugs that prolong QT interval, such as certain antibiotics and antiarrhythmics, increase risk of dangerous heart rhythm changes.
- Enzyme inducers: Medications like rifampin and carbamazepine can reduce haloperidol effectiveness by increasing its metabolism.
- Enzyme inhibitors: Drugs such as fluoxetine and paroxetine can increase haloperidol levels and side effects.
- Levodopa: Haloperidol can reduce the effectiveness of levodopa used to treat Parkinson’s disease.
- Blood pressure medications: May enhance the blood pressure-lowering effects of antihypertensive drugs.

Dosage
Typical adult dosing varies significantly based on the condition being treated and individual patient response. For schizophrenia and psychosis, initial oral doses typically range from 0.5 to 5 mg two to three times daily, with maintenance doses usually between 1 to 15 mg daily. For Tourette syndrome, lower doses of 0.5 to 5 mg daily are often effective. Elderly patients typically require lower starting doses. Dosage should always be individualized and determined by a healthcare provider based on the patient’s specific condition, response to treatment, and tolerance of side effects.
