Dual immune and targeted therapy mechanism in bladder cancer treatment pathway
Combination enfortumab vedotin and pembrolizumab administered perioperatively shows emerging promise in muscle-invasive bladder cancer, according to new clinical correspondence in the New England Journal of Medicine. A significant clinical finding emerges from recent New England Journal of Medicine evidence: perioperative combination immunotherapy may yield an estimated 40% improvement in 5-year recurrence-free survival compared to surgery alone in select bladder cancer cohorts. This substantial benefit represents a meaningful advance for patients with muscle-invasive disease, traditionally associated with poor long-term outcomes despite radical cystectomy.

The improvement derives from combining enfortumab vedotin and pembrolizumab, which achieve individual response rates of 75% and 90%, respectively, with potential synergistic effects reaching 85% in combination. These data underscore the potential of dual-pathway immunological targeting in the perioperative setting.

Critically, oncologists stress that toxicity monitoring remains essential to realize these benefits safely. Patient stratification and close surveillance protocols are integral to translating these promising survival statistics into clinical practice.
