The Nature Medicine study employed spatial transcriptomics to identify gene expression signatures around amyloid plaques, revealing that microglial responses represent a critical determinant of cognitive outcomes. This cellular phenotyping demonstrates that the presence of Alzheimer’s pathology alone does not guarantee dementia—the brain’s immune response to this pathology plays the decisive role.
