CMS Finalizes Medicaid Work Requirements Affecting Millions of Americans

The Centers for Medicare & Medicaid Services has released final regulations requiring millions of non-disabled Medicaid recipients aged 19-64 without dependent children to demonstrate work activities or risk losing coverage. The policy applies to adults enrolled in state Medicaid expansion programs and establishes a phased implementation timeline across three years, beginning with limited rollout before reaching full requirements.

The final rules provide states with flexibility in program design while maintaining federal oversight. Qualifying activities include traditional employment, job training, education, community service, and caregiving responsibilities. Importantly, the regulations include exemptions for vulnerable populations including pregnant women, primary caregivers, and individuals with serious mental health conditions. States must submit waiver applications demonstrating how requirements align with Medicaid’s core mission of providing healthcare coverage to vulnerable populations.
