How Michigan’s Vaccine Education Requirement Cut School Exemptions in Half—Until the Pandemic

Michigan implemented a pioneering public health strategy in 2015 that required parents seeking vaccination exemptions to attend educational sessions with local health officials. The results were striking: school vaccination waiver rates plummeted from 4.6% to 2.7% between 2015 and 2019, demonstrating the power of face-to-face health education. However, the COVID-19 pandemic reversed these gains significantly. Amid widespread vaccine skepticism and political polarization surrounding pandemic vaccines, waiver rates climbed back above 4% by 2023, erasing nearly a decade of progress. The case illustrates both the promise and the fragility of education-based interventions in public health—showing that sustained institutional trust and consistent messaging are essential for long-term success.
