A comprehensive meta-analysis has quantified a striking neurochemical commonality among anxiety patients: an 8% reduction in cortical choline levels compared to individuals without anxiety. This finding emerged from analysis of 25 magnetic resonance spectroscopy studies involving 712 participants—370 with various anxiety disorders and 342 healthy controls.
Remarkably, this deficit manifests consistently across four distinct anxiety subtypes: generalized anxiety disorder, panic disorder, social anxiety disorder, and specific phobias. The prefrontal cortex demonstrated the most pronounced choline reduction, suggesting this emotion-regulation hub bears particular neurometabolic burden in anxiety conditions. Choline is essential for neuronal membrane function and neurotransmitter production, implying that anxiety disorders may chronically elevate metabolic demand in affected brain regions. This represents the first identified transdiagnostic brain chemistry signature in anxiety research.
