Vitamin D insufficiency has reached epidemic proportions in North America, affecting 40% of Americans and up to 97% of Canadians, according to peer-reviewed epidemiological data reviewed by GMJ. The disparity reflects significant geographic and demographic disparities in vitamin D synthesis capacity. Geographic latitude emerges as the primary determinant, with individuals living 40 degrees north or south of the equator receiving virtually no biogenic vitamin D activity during winter months. Additionally, melanin content in skin significantly influences vitamin D synthesis efficiency. Darker-skinned individuals require 5–10 times more sun exposure than lighter-skinned individuals to produce equivalent active vitamin D levels. These metabolic variations have direct clinical consequences, with chronic deficiency increasing risk for metabolic bone diseases including rickets and osteoporosis.
