First, manufacturers maintain elevated list prices that determine insurance reimbursement rates and overall system costs. Second, manufacturer assistance programs offset patient copayments but do not reduce federal spending or underlying medication expenses. Third, the voluntary nature of the agreement lacks enforcement mechanisms for sustained cost control.
These limitations mean that while individual patients may experience reduced out-of-pocket expenses through assistance programs, federal programs and insurers continue bearing inflated costs. Sustainable price reduction requires mandatory negotiation frameworks rather than voluntary industry partnerships.
Physicians counseling patients on GLP-1 therapy should understand that apparent affordability improvements may mask continued systemic cost escalation.
