What is Febuxostat?
Febuxostat is a prescription medication that belongs to a class of drugs called xanthine oxidase inhibitors. It is primarily used to treat gout by reducing the production of uric acid in the body. This oral medication helps prevent gout attacks and manages chronic hyperuricemia (high levels of uric acid in the blood).
Uses
Febuxostat is approved and commonly prescribed for several conditions related to elevated uric acid levels:
- Chronic management of hyperuricemia in gout patients: The primary indication for preventing recurrent gout attacks by maintaining lower uric acid levels
- Gout prevention in patients who cannot tolerate allopurinol: Alternative treatment for those experiencing adverse reactions to first-line therapy
- Tumor lysis syndrome prevention: Used in cancer patients undergoing chemotherapy to prevent dangerous spikes in uric acid levels
- Chronic kidney disease with hyperuricemia: May be prescribed when kidney function is compromised and uric acid elimination is reduced
- Tophaceous gout management: Helps dissolve existing uric acid deposits (tophi) in joints and soft tissues
How it works
Febuxostat works by selectively inhibiting xanthine oxidase, an enzyme responsible for converting purines into uric acid during the final steps of purine metabolism. By blocking this enzyme, the medication significantly reduces the production of uric acid in the body, leading to lower serum uric acid concentrations. This reduction helps prevent the formation of uric acid crystals in joints and tissues, which cause the painful inflammation characteristic of gout attacks.
Side effects
Common side effects include:
- Liver function abnormalities
- Nausea and digestive upset
- Joint pain and muscle aches
- Rash or skin reactions
- Diarrhea
- Headache
- Dizziness
- Fatigue
Serious side effects requiring immediate medical attention:
- Severe liver injury or hepatitis
- Cardiovascular events (heart attack, stroke)
- Severe allergic reactions with skin peeling
- Signs of liver failure (yellowing of skin or eyes, dark urine)
- Unusual bleeding or bruising
Warnings and precautions
- Cardiovascular risk: Studies suggest an increased risk of cardiovascular death compared to allopurinol, particularly in patients with existing heart disease
- Liver monitoring required: Regular liver function tests are essential as febuxostat can cause severe hepatotoxicity
- Pregnancy and nursing: Limited safety data available; should only be used if potential benefits justify risks to the fetus
- Kidney disease considerations: While dose adjustment may not be necessary in mild to moderate kidney impairment, close monitoring is recommended
- Gout flare prevention: May initially trigger gout attacks; concurrent anti-inflammatory therapy is often recommended during initiation
Interactions
Febuxostat can interact with several medications and substances:
- Azathioprine and mercaptopurine: Potentially life-threatening interaction as febuxostat can increase levels of these immunosuppressive drugs
- Theophylline: May increase theophylline levels, requiring dose adjustments and monitoring
- Warfarin: Can enhance anticoagulant effects, increasing bleeding risk
- Colchicine: Often used together for gout management but requires careful dosing
- NSAIDs: Commonly co-prescribed but may increase risk of kidney problems
- Antacids: May reduce febuxostat absorption when taken simultaneously

Dosage
The typical starting dose for adults is 40 mg once daily, taken orally with or without food. If serum uric acid levels remain above target (typically 6 mg/dL) after 2 weeks, the dose may be increased to 80 mg once daily. The maximum recommended dose is 80 mg daily. For patients with mild to moderate kidney impairment, no initial dose adjustment is typically required, though monitoring is important. Dosage should always be individualized and determined by a healthcare provider based on patient response, kidney function, and serum uric acid levels. It may take several months to achieve target uric acid levels and see clinical benefits.
