What is Irbesartan?

Irbesartan is a prescription medication belonging to a class of drugs called angiotensin receptor blockers (ARBs). It is primarily used to treat high blood pressure (hypertension) and to protect the kidneys in people with type 2 diabetes. The medication works by blocking certain receptors in the body that cause blood vessels to constrict, helping to lower blood pressure and reduce strain on the cardiovascular system.

Uses

  • High blood pressure (hypertension): Irbesartan is commonly prescribed as a first-line treatment for elevated blood pressure, either alone or in combination with other antihypertensive medications.
  • Diabetic kidney disease (diabetic nephropathy): The medication helps protect kidney function in patients with type 2 diabetes who have high blood pressure and protein in their urine.
  • Heart failure: In some cases, irbesartan may be used as part of a comprehensive treatment plan for heart failure when other medications are not suitable.
  • Cardiovascular risk reduction: By controlling blood pressure, irbesartan helps reduce the risk of heart attacks, strokes, and other cardiovascular complications.
  • Left ventricular hypertrophy: The medication may help reduce enlargement of the heart’s main pumping chamber associated with high blood pressure.

How it works

Irbesartan blocks angiotensin II receptors, specifically the AT1 receptor subtype, which prevents the hormone angiotensin II from binding and exerting its effects. Angiotensin II normally causes blood vessels to narrow and promotes sodium retention, both of which increase blood pressure. By blocking these receptors, irbesartan allows blood vessels to relax and widen, while also reducing fluid retention, resulting in lower blood pressure and decreased workload on the heart.

Side effects

Common side effects:

  • Dizziness or lightheadedness
  • Fatigue or weakness
  • Upper respiratory tract infections
  • Diarrhea
  • Heartburn or indigestion
  • Muscle or joint pain
  • Headache
  • Nausea

  • Severe dizziness or fainting
  • Signs of high potassium levels (irregular heartbeat, muscle weakness)
  • Kidney problems (changes in urination, swelling)
  • Allergic reactions (rash, swelling, difficulty breathing)
  • Severe dehydration or electrolyte imbalances

Warnings and precautions

  • Pregnancy: Irbesartan is contraindicated during pregnancy, especially in the second and third trimesters, as it can cause serious harm to the developing fetus, including kidney problems and death.
  • Kidney disease: Patients with severe kidney impairment should use irbesartan with caution, as the medication can worsen kidney function in some cases.
  • Liver disease: Dose adjustments may be necessary for patients with liver problems, as the medication is metabolized by the liver.
  • Volume depletion: Patients who are dehydrated or taking diuretics should be monitored closely, as irbesartan can cause excessive blood pressure lowering.
  • Elderly patients: Older adults may be more sensitive to the blood pressure-lowering effects and may require lower starting doses.

Interactions

  • Potassium supplements and salt substitutes: Can increase the risk of dangerous potassium levels when combined with irbesartan.
  • Lithium: Irbesartan may increase lithium levels in the blood, potentially leading to lithium toxicity.
  • NSAIDs (ibuprofen, naproxen): May reduce the blood pressure-lowering effects of irbesartan and increase kidney damage risk.
  • Other blood pressure medications: Combining with other antihypertensive drugs may cause excessive blood pressure reduction.
  • Diuretics: Can enhance the blood pressure-lowering effect, potentially causing hypotension, especially when starting treatment.
  • ACE inhibitors: Should generally not be used together due to increased risk of kidney problems and high potassium levels.
  • Diabetes medications: May enhance blood sugar-lowering effects, requiring monitoring of glucose levels.
  • Alcohol: Can increase the blood pressure-lowering effects and may cause dizziness or fainting.

Dosage

The typical starting dose for adults with high blood pressure is 150 mg once daily, which may be increased to 300 mg once daily if needed. For diabetic kidney disease, the usual dose is 300 mg once daily. Patients who are elderly, have kidney problems, or are taking diuretics may require lower starting doses of 75 mg daily. Irbesartan can be taken with or without food, and it’s important to take it at the same time each day. Dosage should always be individualized and determined by a healthcare provider based on the patient’s condition, response to treatment, and other medications being taken.
