But microglia also have protective modes, in which they clear myelin debris and actively support remyelination — the repair process MS medicine desperately wants to unlock.

This dual nature defines the strategy the review maps: not destroying microglia, but modulating them — pushing them from damaging states into reparative ones. Multiple drug candidates targeting microglial signalling pathways are moving through research pipelines, aiming precisely at the progressive disease phase where today’s immunotherapies fall short.

MS affects millions worldwide, typically striking young adults. Treatments that could slow progression and promote repair — rather than only preventing relapses — would transform lives. Understanding microglia is how medicine gets there.
