Gepants Emerge as New Therapeutic Avenue for Vestibular Migraine Management

Researchers publishing in Otolaryngology–Head and Neck Surgery have identified gepants as a promising treatment class for vestibular migraine, a disabling condition characterized by the co-occurrence of migraine headaches with balance disturbances and dizziness. Gepants, small-molecule antagonists of the calcitonin gene-related peptide (CGRP) receptor, work by blocking CGRP—a neuropeptide central to migraine pathophysiology—thereby interrupting the neuroinflammatory cascade that precipitates migraine attacks.

The study evaluated both oral and intranasal formulations, with both demonstrating strong efficacy and tolerability profiles. These findings represent a significant advancement for vestibular migraine patients, particularly those who have experienced inadequate symptom control with conventional migraine therapies. The dual-route availability of gepants offers clinicians flexibility in treatment selection, potentially improving outcomes for this underrecognized migraine subtype.
