Understanding MTHFR genetics and riboflavin’s role requires three key takeaways. First, the C677T TT variant encodes a thermolabile enzyme that loses its essential B2 cofactor more readily than wild-type variants, creating a metabolic vulnerability. Second, clinical trials demonstrate that 1.6 mg daily riboflavin—achievable through diet or modest supplementation—produces a 13 mmHg systolic blood pressure reduction in TT carriers exclusively, with no effect in other genotypes. Third, while the mechanistic basis and clinical outcome data are robust, current evidence derives from a single research institution, making independent replication essential before recommending genotype-guided riboflavin supplementation in clinical practice.
