The POISE-3 trial provides actionable evidence that should reshape surgical practice. Four critical takeaways emerge from this large-scale international study: first, tranexamic acid demonstrably reduces bleeding complications by 11% in major noncardiac surgery; second, patients require fewer blood transfusions, reducing associated risks and healthcare costs; third, the medication poses no increased risk of dangerous blood clots; and fourth, at under $5 per dose, it represents an accessible intervention globally. For surgical teams and healthcare administrators, these findings suggest tranexamic acid should be considered standard prophylaxis in major procedures. For patients facing major surgery, understanding this evidence-based option may provide reassurance about bleeding risk reduction. The medication’s safety profile and minimal cost make it particularly valuable in low-resource settings where blood product availability is limited.
