Hearing loss is the most prevalent sensory disability globally: 1.5 billion people have some degree of hearing loss, of whom 430 million require rehabilitation — yet only 17% of those who need hearing aids have access to them (WHO Global Hearing Report 2021). Cochlear implants transform outcomes
Cochlear implants — for eligible individuals with profound sensorineural hearing loss — provide functional hearing and enable spoken language development in children. They are highly cost-effective but remain inaccessible in most LMICs.

Glossary of key terms

Disabling hearing loss
WHO
WHO defines disabling hearing loss as hearing loss greater than 35 dB in the better ear in adults, and greater than 30 dB in children. This level of hearing loss significantly impairs communication.
Sensorineural hearing loss (SNHL)
WHO/AAO
Hearing loss from damage to the inner ear (cochlea) or auditory nerve — the most common type in adults. Causes: noise, ageing (presbycusis), infections, ototoxic drugs, genetic conditions. Usually permanent; treated with hearing aids or cochlear implants.
Conductive hearing loss
WHO
Hearing loss from problems in the outer or middle ear (ear canal, eardrum, ossicles) that prevent sound reaching the inner ear. Causes: earwax impaction, otitis media, perforated eardrum. Often treatable medically or surgically.
Cochlear implant
WHO/FDA
An electronic device surgically implanted in the cochlea that directly stimulates the auditory nerve — bypassing damaged hair cells. Provides functional hearing in people with profound-severe SNHL. Most effective when implanted early in childhood (before age 2 for language development).
Noise-induced hearing loss (NIHL)
WHO/NIOSH
Permanent sensorineural hearing loss from exposure to excessive sound levels. Caused by one-time loud noise (acoustic trauma) or prolonged moderate noise exposure. Entirely preventable with hearing protection and safe listening practices. Irreversible once established.

WHO's integrated ear and hearing care framework: High-quality, Equitable, Accessible and Reliable hearing services across the life course — integrated into primary health care. Five priority areas: newborn and infant hearing; child and adolescent hearing; adult and elderly hearing; occupational hearing; and ear disease management.
