The UK Health Security Agency has released comprehensive guidance addressing a significant clinical challenge: managing patients with incomplete or uncertain vaccination histories. This evidence-based framework provides healthcare professionals with clear protocols for a population affecting millions globally, including those who have lost vaccination documentation, received immunisations abroad, or have gaps in their medical records.

The guidance establishes a conservative, safety-first approach that assumes no previous vaccination when documentation is uncertain. This ensures patients receive adequate protection against vaccine-preventable diseases while minimising potential risks. The recommendations cover various scenarios, from complete absence of records to partial documentation, with tailored protocols for different vaccine types, including special timing considerations for live vaccines.
