New evidence-based recommendations from the UK Health Security Agency establish that patients with uncertain or incomplete immunisation records should receive a complete primary vaccination series. This data-driven approach reflects a safety-first paradigm shift in vaccination management, ensuring comprehensive protection against preventable diseases.

The guidance emphasises that healthcare practitioners should not assume previous vaccination when documentation is unclear or unavailable. By implementing this conservative standard across clinical settings, the recommendations address gaps in immunisation coverage that previously existed due to inconsistent record-keeping practices. The framework applies universally to patients with lost records, incomplete documentation, or unclear vaccination timing.

This standardised approach strengthens population immunity while reducing uncertainty in clinical decision-making, ultimately improving public health outcomes across diverse healthcare settings.
