New research from Drug and Alcohol Dependence (2025) explains a common observation: why alcohol affects people differently even when they consume the same amount. Using fMRI imaging, scientists found that individual variation in brain network fragmentation—not just blood alcohol concentration—determines how intoxicated a person feels.
Three key insights emerge from this study. First, brain networks do fragment at legal alcohol limits, but the extent of fragmentation varies substantially between individuals. Second, this neural fragmentation correlates directly with subjective intoxication intensity, independent of BAC measurements alone. Third, this neurobiological variation explains why some people experience significant impairment while others feel relatively unaffected at identical legal alcohol limits.
These findings suggest that individual alcohol sensitivity has a measurable neurological basis, potentially informing future personalized approaches to assessing impairment and understanding alcohol vulnerability.
