What Is Pica?
Pica is an eating disorder in which a person repeatedly consumes things with no nutritional value — earth, sand, clay, chalk, paper, ice. It occurs most often in young children, in pregnancy, and in people with iron deficiency or developmental conditions. The name comes from the Latin for magpie — the bird that eats anything. Pica is frequently hidden: children eat sand or soil unobserved, and parents may not know until complications force the issue.
How Sand Causes a Surgical Emergency
Ingested sand does not digest. It accumulates, and can physically obstruct the appendix — triggering inflammation exactly as a faecalith (hardened stool) would, but with an added complication: a bowel burdened with sediment. In this case, the sand ingestion converted a common childhood emergency into a complicated one, altering both the surgery and the aftercare.
The Lessons
For clinicians: unusual appendicitis presentations warrant asking what the patient may have ingested, and imaging findings of dense material deserve follow-through. For families: pica is a medical condition, not misbehaviour — it warrants evaluation, including screening for iron deficiency and other causes, before it produces emergencies like this one.
