Musculoskeletal conditions are the leading cause of disability worldwide: 1.71 billion people live with musculoskeletal conditions — including low back pain (the single largest cause of disability globally), neck pain, fractures, injuries, osteoarthritis, rheumatoid arthritis and osteoporosis (WHO Global Report on Musculoskeletal Health 2023). The burden is rising sharply with population ageing and physical inactivity — and the overwhelming majority of sufferers live in LMICs where access to rehabilitation and effective pain management is virtually absent. The WHO Rehabilitation 2030 initiative and the Global Alliance for Musculoskeletal Health (G-MUSC) are the primary international frameworks for response.

Rehabilitation — physical therapy, exercise, pain management — is the central treatment strategy for most musculoskeletal conditions. Yet access to rehabilitation is severely limited in LMICs where 80% of the burden falls.
Prevention possible
Regular physical activity prevents many musculoskeletal conditions — reducing osteoporosis, osteoarthritis progression and low back pain. Ergonomic workplaces, fall prevention and fracture liaison services are also evidence-based.
WHO G-MUSC and Global Report
The WHO Global Report on Musculoskeletal Health (2023) — the first comprehensive global assessment — provides the evidence base for national musculoskeletal health policies, calling for integration into primary health care and UHC packages.

Glossary of key terms

Low back pain (LBP)
WHO/GBD
Pain in the region between the lower rib margin and the buttock creases — the world's leading cause of disability. Most acute LBP resolves within 6 weeks; chronic LBP (>12 weeks) is driven by biological, psychological and social factors.
Osteoarthritis (OA)
WHO/OARSI
A chronic joint condition characterised by progressive cartilage degradation, bone remodelling and pain — predominantly of knee, hip and hand joints. Affects 528 million people globally. Risk factors: ageing, obesity, joint injury. Management: exercise, weight loss, analgesia, joint replacement.
Rheumatoid arthritis (RA)
WHO/EULAR
A chronic, systemic autoimmune inflammatory condition predominantly affecting joints — causing pain, swelling, stiffness and joint destruction. Treated with DMARDs (methotrexate) and biologics (TNF inhibitors, IL-6 inhibitors, JAK inhibitors). Early aggressive treatment prevents joint destruction.
Osteoporosis
WHO/IOF
A systemic skeletal disease characterised by low bone mass and microarchitectural deterioration — increasing fragility fracture risk. Affects approximately 10% of women over 60. Prevented by calcium, vitamin D, physical activity; treated with bisphosphonates, denosumab, teriparatide.
Fragility fracture
IOF/WHO
A fracture from minimal trauma (e.g. a fall from standing height) — indicating underlying osteoporosis. Hip fractures are the most serious: 30% of hip fracture patients die within 1 year. Fracture Liaison Services (FLS) prevent secondary fractures.
DMARDs
EULAR/ACR
Disease-Modifying Anti-Rheumatic Drugs — medicines that slow or stop joint damage in inflammatory arthritis (RA, psoriatic arthritis, ankylosing spondylitis). Conventional DMARDs (methotrexate, hydroxychloroquine); biologic DMARDs (TNF inhibitors, IL-6 inhibitors); JAK inhibitors.
