Modern automated haematology analysers are fast, accurate — and sometimes wrong. A case report by Georgian haematologist Giorgi Tkeshelashvili, published in the Georgian Medical Journal, illustrates why human expertise still matters when machines give unexpected results.
The Case: Familial Pelger-Hüet Anomaly
Pelger-Hüet anomaly is a benign inherited condition affecting white blood cells, giving them an unusual appearance that can mimic more serious conditions such as left-shifted neutrophilia (a sign of severe infection) or myelodysplastic syndrome. Automated counters are particularly prone to misclassifying these cells.
The Lesson
This case demonstrates that when automated results don’t fit the clinical picture, a physician’s eye under the microscope remains irreplaceable. It’s a reminder that technology supports — but does not replace — clinical judgement.
