Breast cancer is the most common cancer in women worldwide — and since 2021, the most common cancer overall: 2.3 million new cases and 685,000 deaths in 2020 — with incidence rising in every world region, including LMICs where most deaths occur (IARC/WHO). Triple-negative is most aggressive
Triple-negative breast cancer (TNBC) — accounting for 10-15% of cases — lacks hormone receptors and HER2 overexpression, making it unresponsive to hormone therapy and targeted therapy. It is more aggressive and disproportionately affects younger women and those of African ancestry.
Risk factors
Known risk factors include: older age, female sex, inherited BRCA1/BRCA2 gene mutations, dense breast tissue, early menarche, late menopause, nulliparity, late first pregnancy, hormone therapy, alcohol and obesity. 50% of breast cancers occur in women with no identified risk factor.
Men get breast cancer too
Approximately 0.5-1% of breast cancers occur in men — approximately 2,650 cases per year in the US. Male breast cancer has worse outcomes partly from delayed diagnosis due to low awareness.

Glossary of key terms

BRCA1/BRCA2
NCCN/WHO
Tumour suppressor genes — mutations in which dramatically increase lifetime breast cancer risk (45-85% for BRCA1 carriers). Hereditary breast and ovarian cancer syndrome (HBOC). Genetic testing and enhanced surveillance or prophylactic surgery are recommended for carriers.
Triple-negative breast cancer (TNBC)
WHO/ASCO
Breast cancer lacking oestrogen receptor (ER), progesterone receptor (PR) and HER2 overexpression. Unresponsive to hormone therapy and HER2-targeted therapy. More aggressive, more common in younger women and women of African ancestry. Treated with chemotherapy and immunotherapy.
HER2-positive breast cancer
WHO/ASCO
Breast cancer overexpressing the HER2 protein — approximately 15-20% of breast cancers. Previously very aggressive, now highly treatable with targeted therapies (trastuzumab/Herceptin, pertuzumab, TDM1). Access to HER2-targeted therapy in LMICs remains severely limited.
Mammography
WHO/IARC
X-ray imaging of the breast for cancer screening. Evidence-based for women aged 50-69 (biennial). Reduces breast cancer mortality by approximately 20%. Requires radiological infrastructure and follow-up capacity. Not appropriate for universal screening in all LMIC contexts.
Clinical breast examination (CBE)
WHO/GBCI
Physical examination of the breast by a trained healthcare worker — a lower-technology screening approach recommended by WHO for LMICs without mammography infrastructure. Detects palpable tumours; less sensitive than mammography for early-stage cancers.
