No specific treatment
There is no specific antiviral for dengue. Management is supportive — hydration, fever control and monitoring for warning signs of severe dengue. Prevention is through vector control and vaccination.
New vaccines
TAK-003 (Qdenga, Takeda, 2023) and CYD-TDV (Dengvaxia, Sanofi) are licensed. WHO recommends Dengvaxia only for seropositive individuals; Qdenga appears effective regardless of prior exposure.
Wolbachia breakthrough
Wolbachia bacteria introduced into Aedes mosquitoes reduce dengue transmission capacity. A Phase III trial (Indonesia) showed 77% reduction in dengue incidence — a potential game-changer.

Glossary of key terms

DENV
WHO
Dengue virus — a Flavivirus with four distinct serotypes (DENV-1 to DENV-4). Infection with one serotype confers lifelong immunity to that serotype but only partial, temporary cross-immunity to others.
Aedes aegypti
WHO/CDC
The primary dengue vector — an urban mosquito that breeds in small standing water and bites predominantly during the day. Aedes albopictus is a secondary vector now expanding into temperate climates.
Severe dengue
WHO
Previously called dengue haemorrhagic fever — characterised by plasma leakage, haemorrhage and organ impairment. Typically occurs with second infections via antibody-dependent enhancement (ADE). Case fatality >2.5% without treatment.
Warning signs
WHO
Clinical features indicating progression to severe dengue: severe abdominal pain, persistent vomiting, fluid accumulation, mucosal bleeding, lethargy, liver enlargement, rapid fall in platelet count. Recognition is the key clinical skill.
Wolbachia biocontrol
World Mosquito Program
Naturally occurring bacteria introduced into Aedes mosquitoes to reduce their capacity to transmit dengue. Phase III trial showed 77% reduction in dengue incidence. Spreading through mosquito populations within months of release.
Dengvaxia / Qdenga
WHO/Sanofi/Takeda
Two licensed dengue vaccines. WHO recommends Dengvaxia only for seropositive individuals (prior dengue increases severe dengue risk in seronegatives). Qdenga (TAK-003, 2023) appears effective regardless of prior dengue exposure.
