Two events moved talc from stale controversy to live science. In July 2024 the WHO's cancer agency upgraded talc from 'possibly' (Group 2B, its 2010 verdict) to 'probably carcinogenic to humans' (Group 2A) — resting on limited but consistent human evidence for ovarian cancer with perineal powder use, sufficient evidence in experimental animals, and strong mechanistic evidence. Weeks earlier, an analysis of over 50,000 women in the prospective Sister Study reported that frequent, long-term genital talc use was associated with meaningfully elevated ovarian cancer risk — notable because it was designed to address the recall-bias criticism that had haunted the field, and still found the signal. Neither finding is proof of causation; together they moved the needle from 'probably noise' toward 'probably something'.
WHY THE EVIDENCE STAYED CONTESTED FOR FORTY YEARS
The epidemiology split along study design for decades: case-control studies — asking women with ovarian cancer to recall powder use — consistently found elevated risk, but are vulnerable to recall bias amplified by publicised litigation; prospective cohorts, which record exposure before disease, mostly found weaker or statistically non-significant associations, including a 2020 pooled analysis of a quarter-million women whose overall estimate hovered just above null. The candidate mechanism — talc particles migrating up the reproductive tract and driving chronic ovarian inflammation — is plausible and supported by particle findings in tissue, but unproven as a causal chain. Reasonable scientists read this record differently; what nobody disputes is that if the risk is real, it is modest in relative terms against a baseline lifetime ovarian cancer risk of roughly 1-2%.
THE ASBESTOS SHADOW: the question inside the question
Talc and asbestos are geological neighbours, mined from adjacent or intermingled deposits — so every talc study carries an identification problem: is any risk from talc itself, or from asbestos contamination of historical talc products? IARC noted contamination could not be excluded in most human studies, and investigative reporting plus litigation discovery produced internal company documents showing awareness of asbestos detections in talc supplies across decades — the single most damaging material in the courtroom record. The 2024 classification formally addressed asbestos-free talc, but the historical exposure the epidemiology measures is the products people actually used. For inhaled occupational exposure, asbestos-contaminated talc's link to mesothelioma is not controversial at all.
THE LITIGATION MACHINE: science in an adversarial engine
Johnson and Johnson faced on the order of sixty thousand claims that talc products caused ovarian cancer or mesothelioma, producing billion-dollar verdicts, two rejected attempts to resolve liability through subsidiary bankruptcy, a 700-million-dollar settlement with US states over marketing, a 6.5-billion-dollar proposal, and — in July 2026 — a multibillion-dollar settlement agreement, all while the company maintained its products were safe and cited the cohort evidence. The company stopped selling talc-based baby powder in North America in 2020 and worldwide in 2023, reformulating with cornstarch and describing the decision as commercial. Litigation of this scale distorts the science it consumes in both directions — plaintiff-funded and defence-funded studies, expert witnesses on retainer — and reading the talc literature requires checking the conflict-of-interest statements line by line.
GENUINELY OPEN: dose, subtype, mechanism
What remains unresolved after four decades: whether the association is causal at all (the Sister Study strengthens but does not settle it); the dose-response shape — frequency and duration matter in the positive studies, but thresholds are undefined; subtype specificity — signals concentrate in serous tumours, consistent with a migration-inflammation mechanism, but inconsistently; and whether modern, asbestos-verified cosmetic talc carries any of the risk measured from historical products. A 2024 US regulatory proposal to standardise asbestos testing of talc cosmetics addresses the contamination question prospectively — which will, over decades, quietly answer part of the historical one.
PRACTICAL BOTTOM LINE
The exposure is trivially avoidable: cornstarch-based powders do the same job, are used by the reformulated products themselves, and carry none of the question — for genital use in particular, no one needs talc. Past use is not a sentence: any absolute risk increase is small against a low baseline, and stopping ends the exposure; there is no screening implication and no reason for alarm. Occupational inhalation of talc dust belongs to workplace protection rules regardless of the ovarian question. And claims in either absolute direction — 'talc definitively causes ovarian cancer' or 'the science fully cleared it' — are litigation positions, not evidence summaries.
